DIOCESAN FORMS

Incardination and Ordination certificates are printed on wide-bordered certificate paper. A sample certificate is located here.
The General Secretary will provide the bishop with a prepared certificate, including printed ordination date. 
The certificate will be mailed to the Bishop for his or her signature. 
An envelope will be included for sending the signed certificate to the new deacon or priest.

LETTERS OF APPOINTMENT
Letters of Appointment for Canons, Regional Deans, and Archdeacons
are available through the General Secretary.  Below is a sample letter appointing a Canon.
