Disposition of Remains

Use this standardized form to designate who is to handle your funeral arrangements, as well as written instructions for the disposition of your remains.   All too often in large families there is dissent and contention at the time of death, with family members creating tensions over final arrangements.   By completing this form while you are of sound mind, and making sure there is a copy on file with your intended funeral home, such written instructions not only define what is to be done, but makes life much easier on the funeral director.

This form would be of particular usefulness to those who are transgendered, and whose families are intent to have them dressed and prepared for burial in a manner that is opposite to the gender with which they identity.
